Uman underwent a routine, out-patient endoscopy performed by a Jacksonville doctor who held himself out to be an expert in the field of gastroenterology.  Unfortunately, because neither the doctor nor his nursing staff took the time to read the medical records provided to them by Mr. Uman, the Jacksonville doctor did not learn until it was too late that Mr. Uman could not be appropriately sedated with Demerol. As a consequence of his negligent oversight, and for other reasons still unknown, the doctor and his nursing staff administered a huge overdose of Versed, Demerol, and concentrated iodine causing a multi-system failure, including acute kidney failure, thrombocytopenia, and serious injuries to his liver, pancreas, and other organs. After an extensive hospital stay, and despite the odds against him, Jon D. Uman survived.

When he later began to investigate what went wrong at the Jacksonville outpatient clinic, he learned several interesting things: Although the doctor and the nursing staff  recorded the total amount of Versed and Demerol administered, they failed to record the time or amount of any  individual intravenous doses.  Moreover, there was absolutely no record of the time or amount of iodine administered. Most disturbing, there was no record of vital signs for long periods of time during the procedure. Accordingly, it is still unknown whether Jon D. Uman suffered shock, respiratory distress, hypotension, pulmonary failure, or other life threatening complications during the procedure.
